The bioscans do pick up pretty much everything about a character, so they would pick up altered human traits or cyborg traits. If a character lies about their physiology and the truth comes out in the bioscan, it wouldn't cause serious problems - the Orientation team would likely ask the character if they have a reason for hiding what they are, and would be extended an offer to keep that part of their public-record bio stricken/private. It would be treated more as a private health matter similar to a medical record unless the team got the feeling the character was lying for dangerous reasons. If the reasons are related to dysphoria or some other concern, the character may also be referred to a free therapist or counsellor.

The translation magic can be as flawless or flawed as you like, and it can be variable. As a mod, I'm more concerned with players having fun than universal/perfect game mechanics, and I generally explain it away as magic affecting different people differently.
