Thanks for your question! Death is actually permanent in Riverview, and any character who dies would have to be re-applied for fresh from canon. However, just like in real life, people can technically die and be revived afterward, such as when they are brought in either dying or immediately following death. Because of the level of tech in the Quarantine, the length of time that can pass between death and when people are no longer able to be revived is longer than it would be in real life.

Generally, if a character whose vitals have stopped (no heartbeat, no breathing, no brain activity), you can assume that the character can be revived as long as they get to the hospital within about 2 to 3 hours.

In the case of a character who is exploring and could not be brought back to the city in that length of time, the player in question could contact me and, depending on the circumstances, we could possibly work out something alternate on a case-by-case basis, such as the character being revived but permanently damaged in some way that cannot be corrected.
